Terms and Conditions
Introductory Provisions
1.1
These Terms and Conditions of FamilyHub services (“Terms”) govern the rights and obligations of the contractual parties, Family Hub Nusle s.r.o., Company ID No.: 03547957, with its registered office at Vlastislavova 152/4, Nusle, 140 00 Prague 4, registered in the Commercial Register maintained by the Municipal Court in Prague, File No.: C 231918 (“Provider” or “we”), and you as a person requesting or using the services of the Provider (hereinafter referred to as the “Client” or “you”).

1.3
Our mutual rights and obligations arise from the Framework Agreement on the Provision of FamilyHub Services (“Agreement”). These Terms are an integral part of the Agreement.
1.4
The contractual relationship between the Provider and the Client is governed by the Agreement, these Terms, and other attachments to the Agreement.
1.5
If the Client is under 15 years of age, the Agreement is concluded with the Client’s legal representative in written form. For Clients under 18 years of age, the consent of the legal representative is required.
Definitions
2.1
“Price List” means the standard price list of the Provider’s Services, available on the Provider’s website.
2.2
“Civil Code” means Act No. 89/2012 Coll., the Civil Code, as amended.
2.3
“Order” means an individual contract for the provision of Services within a Session concluded via the Portal.
2.4
“Portal” means the Provider’s web portal for managing Orders and user accounts.
2.5
“Session” includes consultations, workshops, lectures, and other Services provided by a Therapist at a designated time and place.
2.6
“Services” means the allocation of premises and the Therapist’s capacity, preparation of the Therapist, and consultations in the areas of social, educational, motivational, and therapeutic support.
2.7
“Therapist” means an employee of the Provider delivering the Services.
User Account and Portal
3.1
Access to the Portal is provided based on the Agreement concluded during registration and acceptance of these Terms.
3.2
The Agreement is concluded for an indefinite period unless otherwise specified. Each party may terminate the Agreement with immediate effect.
3.3
The Client is responsible for the accuracy and maintenance of the data entered during registration.
3.4
Login credentials are confidential. The Client is responsible for their use and for notifying the Provider of any potential misuse.
3.5
The Provider may modify or discontinue the operation of the Portal at any time.
Orders and Conditions of Service Provision
4.1
The Client submits a request for Services by filling in the required information in the Portal.
4.2
Confirmation of the request requires payment of the Service fee.
4.3
Upon acceptance of the request, an Order arises, which the Provider confirms by email.
4.4
Cancellation of a Session at least 48 hours before the scheduled time: 100% refund of the fee. Later cancellation or no-show is non-refundable.
4.5
Any refunded amount is credited in the form of credit, which may be used for other Services or paid out to a bank account within 14 days.
4.6
The standard duration of a Session is 50 minutes unless otherwise agreed.
4.7
The Provider reserves the right to cancel a Session; in such a case, the fee will be refunded in full.
Specification of Services and Liability
5.1
The Services are not medical, psychological, or psychiatric services and do not substitute for professional medical care.
5.2
In case of crisis situations, please call the Mental Health First Aid Line 116 123 or the emergency number 112.
5.3
The Provider bears no responsibility for the Client’s health condition or for their actions before or after the Session.
5.4
If the Therapist identifies a need for professional help, they will recommend it to the Client.
5.5
If the Provider becomes aware of suspected criminal activity, it may inform the law enforcement authorities.
Your Obligations
6.1
The Client is obliged to provide the cooperation necessary for the proper provision of the Services.
6.2
The Client may not assign rights or obligations under the Agreement to third parties without the Provider’s consent.
6.3
It is forbidden to bring dangerous or sharp objects into the Provider’s premises.
Data Protection
7.1
The processing of personal data is governed by the Privacy Policy.
7.2
The Provider maintains confidentiality regarding all information communicated by the Client.
7.3
The Therapist may share information for the purpose of quality assurance, development, and continuity of services.
Payment and Pricing Terms
8.1
The price of Services is determined by agreement or according to the Price List, always including VAT.
8.2
Payments are made by card or other methods specified in the Portal.
8.3
In case of late payment, the Provider is entitled to interest on late payments.
Selected Pre-Contractual Information
9.1
The Client agrees to the immediate commencement of performance before the expiration of the withdrawal period.
9.2
A consumer may withdraw from the Agreement within 14 days; however, there is no right to a refund of a proportional part of the fee for Services already provided.
9.3
Withdrawal is made by sending the form as per Annex No. 1.
9.4
If the Client is entitled to a refund, the amount will be refunded within 14 days.
Special Provisions for Distance Contracts
10.1
Conclusion of the Agreement via the web or telephone is free of charge.
10.2
Formulation of Orders and confirmations are conducted electronically.
Rights Concerning Defects
11.1
Complaints regarding the Services should be sent to the Provider’s registered office address or the contacts specified in Article 1.2.
11.2
The Provider shall resolve the complaint within 14 days; if not, the Client may withdraw from the Agreement or request a discount.
Final Provisions
12.1
The Agreement and the Order include all annexes, including these Terms.
12.2
Amendments to the Agreement or the Terms are possible only through written amendments or by email.
12.3
Invalidity of any provision does not affect the validity of the remaining provisions.
12.4
The Provider may unilaterally amend the Terms and the Price List; the new version becomes effective as of the effective date.
12.5
These Terms are effective as of 14 April 2025.
Annex No. 1: Model Withdrawal Form
